The following forms are available if you are unable to apply online, or would rather have a printable version of the application, scholarship, or teacher recommendation forms. Camp Application (pdf) contains application and audition tape/CD information for all camps. If you are applying for more than one camp, please fill out a separate application for each one.

Scholarship Application (pdf) and other forms required for those seeking financial assistance (teacher recommendation form included). Scholarship awards are based on financial need, student talent and needs of the program. Submit your scholarship application at the same time you submit your camp application; only complete applications are considered.

Teacher Recommendation Form (pdf) You do not need to download this form if you have already downloaded the Camp Application packet, as it is included in the packet.
